How to Request Your Mental Health Care Records
If there is a reason you need copies of your mental health care records, discuss it with your therapist. To request your records, submit a signed, written request to the business office specifying which records you need. Subject to applicable law, your records will be provided within 15 days of receiving your request. A reasonable fee may be charged for retrieving and copying your records.
